If you normally stop by our office to pick up checks, our administrative team will contact you to arrange an alternative.
If you are looking to file a claim for reimbursement, we encourage you to file online through your participant portal or fax your reimbursement request form and documentation to 844-859-7308 (DO NOT include a “1” in front of the “844”). Click here to learn more about how to file a claim.
