To protect your personal health information (PHI) we request that you DO NOT email claims, documentation, or HR forms to GGA.
To submit claims and documentation, you can:
- Online – Submit claims and upload your documentation through the participant portal. Click here for more information on how to access your account.
- Fax – Fax your claims and documentation to (844) 859-7308. DO NOT include a “1” before the (844).
- Upload – Upload your claims and documentation to our ShareFile inbox. You must include your name, employer, and phone number on any uploaded files so they can be processed. Click here to access the upload link.
Employers and HR Administrators
To send HR correspondence and forms, you can:
- Send emails and forms with no PHI to firstname.lastname@example.org.
- Securely upload forms with PHI to our ShareFile inbox. Click here to access the upload link.
